COLLEGE VISIT FORM

I hereby request that my child, __________________________________ be given permission to visit __________________________________ college / university on ___________________. I understand that his / her visit will only be permitted by a school administrator prior to the visit. Furthermore, all class work must be submitted within 2 school days of the college visit. Finally for the college visit to be considered an excused absence from the school this form must be returned to the grade office on the day immediately following the visit with the signature of an admissions representative and stamp of the college or university.
